Parent Driver Form Requirement
If you plan to take your student home from an athletic game or event, a Parent Driver Form must be completed and submitted. This form only needs to be signed once per school year.
You must notify the Athletic Office at least 24 hours in advance everytime you plan to drive your student home from their athletic event.
When leaving the event, parents/guardians must sign their student out with the coach before taking them home.
Students will only be released to parents or guardians who have a completed Parent Driver Form on file and have provided the required advance notice.

Please provide your student’s name and the sport(s) they are participating in. If your student plays multiple sports, you may list all of them. This form will apply to all sports for the school year.
